Ohio Healthy Youth Environments Survey

 
The Ohio Healthy Youth Environment Survey (OHYES!) will be administered at Tolles to all Madison County Juniors, on October 3, 2018, during Tolles time.  The survey is anonymous and is designed to measure the health risk behaviors that contribute to morbidity, mortality, and social problems among Ohio’s youth.  It gathers information on behaviors like alcohol, tobacco, and other drug use, unintentional and intentional injuries like violence, dietary behaviors, physical activity, and related risk and protective factors.  Survey participation is completely voluntary and youth respondents may refuse any questions they do not feel comfortable with.  Parents may also opt their students out of the survey by completing the Opt Out formed, that will arrive at their home in the next day or so.  Opt Out Forms are due back to Tolles Front Office by Tuesday, October 2, 2018.  For more information, visit the Tolles website at www.tollestech.com or call (614)873-4666 extension 4414.
